Post Secondary Survey
The California Department of Education requires information on
the status of students with disabilities who have left high
-school during the previous school year. Please take a few
minutes to answer the post-secondary questions below if you
exited or graduated from the district during the last school
year.
Stay Connected Form
We want to hear about how you are doing. Please fill out the Stay
Connected Form, so the District can connect with you in the near
future. The California Department of Education requires all
districts to gather information about their Post Secondary
students who graduated in a specialized program, and filling out
the Stay Connected Form it will allow the District to connect
with you.
