Each child is required to have a current complete (within the previous 12 months) physical examination within the first 30 days of enrollment, and yearly thereafter.  Staff is available to assist families in locating a doctor, if needed.

The physical exam should be recorded by the doctor on the preschool physical exam form. A lead level, hemoglobin/hematocrit, vision and hearing screen are all part of a complete physical exam.
