Please fill out the following form to request IEP Records for students who have graduated or no longer go to SCUSD schools. If you are a parent looking to obtain a current copy of your child’s IEP, please contact your case manager directly at their school.

Please note: for parents or student’s over 18 years of age, when picking up records we require identification that matches the name on the student records. 

We process IEP Records Requests on a first come, first served basis. Turnaround time is five business days.
