Case Management

Description – Assessments of students with psychological problems including school adjustment problems, truancy, pregnancy, relationship difficulties, physical and mental health conditions.
Case management of students not limited to concerns outlined. Referral and follow-up for dental, medical, vision, hearing, mental health needs.  SST/SART/SARB Attendance. Initial and triennial assessments of students in Special Education. Crisis assistance and prevention.  CSRA (Child Suicide Risk Assessment) for elementary students and SYSR (Screen for Youth Suicide Risk) for secondary students.   Health education includes Family Life classes presented for 4, 5, 6th grade girls and 5, 6th grade boys. Safety committee and emergency preparedness planning consultation by nurse.

Location(s) – As assigned

Hours of Operation – 6 hrs/day

Languages – English, translator secured as needed.

Fees – N/Applicable

Eligibility Requirements – All students are eligible for services provided by the nurse.
