Please note that there is no need to communicate any of the information below. This message is to inform you of what is coming online tomorrow (10/29/21) and will be communicated to District staff, students and volunteers.

Following the October 12th, 2021 SCUSD Board of Education vaccine mandate resolution, which can be read in its entirety on the SCUSD website, our District now has a centralized process in place to collect student, staff and volunteer vaccination records as well as personal belief (including religious) and medical exemption information where applicable. 

We are encouraging the collection of vaccination status and records to be streamlined through our online forms as much as possible to minimize school site impact as we understand the staffing shortages that all of our sites are facing. The new forms and information will launch on the SCUSD website Friday, October 29th. The printed forms in the resource toolkit are pending updates and translations and will be updated by Friday, October 29th. Communications will be sent from the District via email, letters mailed home, text messages, social media and robocall.

We are asking our site leaders to assist with communicating the resolution requirements to our students and families. You and your staff are trusted messengers in your school community who are able to reach our families and address the many questions they may have. We’ve put together this FAQ page to help you get answers to common questions and learn more about what options are available to SCUSD staff and students. 

Further, in an effort to ensure equitable access to reporting vaccination or testing status for all of our students who may need to file hard copies at school sites, we have created a vaccine mandate resource toolkit containing an 11×17 instructional poster which can be printed and posted in school site offices, the vaccination form with translated versions, the personal belief exemption with translated versions and more. We encourage you to take advantage of this toolkit, as it highlights key information.

In the event that a student turns in their forms as a hard copy at the school site, please: 

  • Ask them if they’re able to report their status through our online process.
  • If that is not an option for them, please have staff ensure forms are filled out correctly.
  • Scan and email any forms and records pertaining to the student submission to
  • File all paper forms in the student cumulative folder - health folder (yellow folder). 
  • Treat the information in the forms with confidentiality. 
  • District administrative staff will then ensure that this hard copy data is entered into Informed K-12 and Infinite Campus.
