We would like to share an update with you regarding COVID-19 Vaccination Record Submissions and how this impacts front office staff at your school. 

Following the October 12th, 2021 SCUSD Board of Education vaccine mandate resolution, which can be read in its entirety on the SCUSD website, our District now has a centralized process in place to collect student, staff and volunteer vaccination records as well as personal belief (including religious) and medical exemption information where applicable. 

We are encouraging the collection of vaccination status and records to be streamlined through our online forms as much as possible to minimize school site impact as we understand the staffing shortages that all of our sites are facing. A resource toolkit has been developed for site leaders and staff, including posters, letters, exemption forms and printable hardcopies of the electronic Informed K12 form. 

Communications will be sent from the District via email, letters mailed home, text messages, social media and robocall. Additionally, we have asked Principals to share this information widely with their school communities through your established outreach strategies. School sites are trusted messengers who are able to reach our families and address the many questions they may have. We’ve put together this FAQ page to help you get answers to common questions and learn more about what options are available to SCUSD staff and students. 

Paper Records

If a student/parent/guardian brings you a vaccine card or exemption application, please do the following:

  • Encourage them to attach these records to their online submission (similar to attaching a document to an email)
  • If electronic submission is not viable, provide them with a paper form to complete (translations are available)
    • Collect the completed form along with the vaccine record or exemption application
    • Scan them to 
    • File documents in the cumulative folder/health section
    • Treat the information in the forms with confidentiality
  • Once Health Services receives the scanned records, a central team will process, vet, and document all records in I.C. (see Process Map)

I.C. Compliance Reports

Infinite Campus Compliance rules for COVID-19 Immunizations will go into effect on 1/31/22. At that time, compliance status for COVID will be visible in the Immunization Summary at the top of each student’s Immunizations Tab under Student Information > Health > General

In the meantime, you can run compliance reports to determine the upcoming compliance status for your students using a future effective date in the Generic Immunizations Extract Report. Instructions on how to do this are attached.
