September is National Suicide Prevention Month, a time for us all to pause and reflect on how we can each #BeThe1To prevent death by suicide and save lives.

Know the Facts

Unlike other leading causes of death, suicide is completely preventable. Unfortunately, many still do not know how pervasive deaths by suicide are in the United States:

  • Suicide is the 2nd leading cause of death for young people ages 10-24, and has been increasing every year since 2007 (UC Davis Health, 2021)
     
  • The suicide attempt rate has leapt by as much as half among teenage girls during the coronavirus pandemic (Chiu, 2021)
     
  • Average of 3,703 suicide attempts every day in the U.S. by young people grades 9-12 (CDC, 2020)
     
  • Four out of Five individuals considering suicide give some sign of their intentions, either verbally or behaviorally (CDC, 2020)

Know the Signs

80% of people considering suicide show signs of their intentions – knowing those signs is critical to prevention.  

Risk Factors
  • Mental disorders, particularly mood disorders, schizophrenia, anxiety disorders, and certain personality disorders.
  • Alcohol and other substance use disorders
  • Hopelessness
  • Impulsive and/or aggressive tendencies
  • History of trauma or abuse
  • Major physical illnesses
  • Previous suicide attempt(s)
  • Family history of suicide
  • Recent loss
  • Easy access to lethal means
  • Local clusters of suicide
  • Lack of social support and sense of isolation
  • Stigma associated with asking for help
  • Lack of healthcare, especially mental health and substance abuse treatment.
  • Cultural and religious beliefs, such as the belief that suicide is a noble resolution of a personal dilemma.
  • Exposure to others who have died by suicide (in real life or via the media and Internet).
Warning Signs
  • Talking about wanting to die or to kill themselves
  • Looking for a way to kill themselves, like searching online or buying a gun
  • Talking about feeling hopeless or having no reason to live
  • Talking about feeling trapped or in unbearable pain
  • Talking about being a burden to others
  • Increasing the use of alcohol or drugs
  • Acting anxious or agitated; behaving recklessly
  • Sleeping too little or too much
  • Withdrawing or isolating themselves
  • Showing rage or talking about seeking revenge
  • Extreme mood swings

How to Help

If you observe one or more of these warning signs, especially if the behavior is new, has increased, or seems related to a painful event, loss, or change –step in or speak up.

Say Something

Many of us are uncomfortable talking about suicide, and may even fear that by saying something we could make it worse.  Research shows that asking someone if they are having thoughts of killing themselves is actually more likely to save a life.  

  • Have a list of crisis resources on hand.  Practice what you will say. Plan the conversation for a time when you won’t be in a hurry and can spend time with the person.
     
  • Mention the signs that prompted you to ask about suicide. This makes it clear that you are not asking “out of the blue,” and makes it more difficult for the person to deny that something is bothering them.
     
  • Ask directly about suicide. Talking about suicide does NOT put the idea in someone’s head and usually they are relieved. Asking directly and using the word “suicide” establishes that you and the person at risk are talking about the same thing and lets the person know that you are willing to talk about suicide.
     
  • DO NOT ask in a way that indicates you want “No” for an answer.
     
  • DO NOT tell the person to do it. You may want to shout in frustration or anger, but this is the most dangerous thing you can say.
  • DO NOT promise secrecy. The person may say that they don’t want you to tell anyone that they are suicidal. Say this instead: “I care about you too much to keep a secret like this. You need help and I am here to help you get it.” You may be concerned that they will be upset with you, but when someone’s life is at risk, it is more important to ensure their safety.
Listen

Do not try to solve the person’s problem or tell them you know how you feel.  Let them talk and really hear what they are saying.  Reassure them that you are there for them, that you care about them, and that they are not alone.

  • Listen to the reasons the person has for both living and dying. Validate that they are considering both options and underscore that living is an option for them.
     
  • Let the person know you care. Letting them know that you take their situation seriously, and you are genuinely concerned about them, will go a long way in your effort to support them.
Focus on Safety

Once you have opened the conversation and heard them, it is time to plan for their safety.

  • Ask the person if they have access to any lethal means (weapons, medications, etc) and help remove them from the vicinity. (Another friend, family member or law enforcement agent may be needed to assist with this.)  Do not put yourself in danger; if you are concerned about your own safety, call 911.
     
  • Create a safety plan together. Ask the person what will help keep them safe until they meet with a professional.
Get Help

Know the limits of your own expertise and be help the person see the value in reaching out for professional assistance.

  • Provide the person with the resources you have come prepared with. Call the National Suicide Prevention Lifeline anytime at 1-800-273-8255.
     
  • If you feel the situation is critical, take the person to a nearby Emergency Room or walk-in psychiatric crisis clinic or call 911.

  • Tendencias impulsivas y/o agresivas.
  • Antecedentes de trauma o abuso.
  • Enfermedades físicas graves.
  • Intento(s) de suicidio previo(s).
  • Antecedentes familiares de suicidio.
  • Pérdida reciente.
  • Acceso fácil a medios letales.
  • Grupos locales de suicidio.
  • Falta de apoyo social y sensación de aislamiento.
  • Estigma asociado con pedir ayuda.
  • Falta de atención médica, especialmente salud mental y tratamiento para el abuso de sustancias.
  • Creencias culturales y religiosas, como la creencia de que el suicidio es una noble resolución de un dilema personal.
  • Exposición a otras personas que han muerto por suicidio (en la vida real o a través de los medios de comunicación e Internet).
Señales de Advertencia
  • Hablar de querer morir o de suicidarse
  • Buscar una manera de suicidarse, como buscar en línea o comprar un arma
  • Hablar sobre sentirse desesperado o sobre no tener razón para vivir
  • Hablar sobre sentirse atrapado o sentirse en un dolor insoportable
  • Hablar de ser una carga para los demás

Si observa una o más de estas señales de advertencia, especialmente si el comportamiento es nuevo, ha aumentado o parece estar relacionado con un evento doloroso, pérdida o cambio, intervenga o hable.

Diga Algo

Muchos de nosotros nos sentimos incómodos hablando sobre el suicidio, e incluso podemos temer que al decir algo podríamos empeorarlo.  Las investigaciones  demuestran que preguntarle a alguien si está teniendo pensamientos de suicidio es en realidad más probable que salve una vida.

  • Tenga a la mano una lista de recursos para crisis. Practique lo que dirá. Planifique la conversación para un momento en el que no tendrá prisa y podrá pasar tiempo con la persona.
     
  • Mencione las señales que lo llevaron a preguntar sobre el suicidio. Esto deja en claro que no está preguntando “de la nada”, y hace que sea más difícil para la persona negar que algo le está pasando.
     
  • Pregunte directamente sobre el suicidio. Hablar de suicidio NO pone la idea en la cabeza de alguien y por lo general se sienten aliviados. Preguntar directamente y usar la palabra “suicidio” establece que usted y la persona en riesgo están hablando de lo mismo y le hace saber que usted está dispuesto a hablar sobre el suicidio.
     
  • NO pregunte de una manera que indique que desea un “No” por respuesta.
     
  • NO le diga a la persona que lo haga. Es posible que usted desee gritar de frustración o enojo, pero esto es lo más peligroso que puede decir.
  • NO prometas mantenerlo en secreto. La persona puede decir que no quiere que le digas a nadie que tiene pensamientos suicidas. Diga esto en su lugar: “Me preocupo demasiado por ti como para mantener un secreto como este. Necesitas ayuda y estoy aquí para ayudarte a conseguirla”. Es posible que le preocupe que se molesten con usted, pero cuando la vida de alguien está en riesgo, es más importante garantizar su seguridad.
Escuche

No trate de resolver el problema de la persona ni de decirle que usted sabe cómo se siente. Déjelos hablar y escuche realmente lo que están diciendo. Asegúreles que usted está ahí para ellos, que se preocupa por ellos y que no están solos.

  • Escuche las razones que la persona tiene tanto como para vivir como para morir. Validar que están considerando ambas opciones y subrayar que vivir es una opción para ellos.
     
  • Hágale saber a la persona que le importa. Hacerles saber que se toma en serio su situación, y que está genuinamente preocupado por ellos, será de gran ayuda en su esfuerzo por apoyarlos.

Tháng 9 là Tháng Ngăn Ngừa Tự Tử Toàn Quốc, một thời gian cho tất cả chúng ta ngưng và phản ánh việc chúng ta có thể làm thế nào cho mỗi #BeThe1 ngăn chặn tình trạng tử vong do tự tử và cứu sinh mạng. 

Biết Các Sự Cố

Không như những nguyên nhân tử vong dẫn đầu khác, tự tử hoàn toàn có thể ngăn chặn được.  Thật không may mắn, nhiều người vẫn không biết những cái chết phổ biến do tự tử như thế nào tại Hoa Kỳ:

  • Tự tử là nguyên nhân tử vong thứ 2 cho giới trẻ từ 10-24 tuổi, và đang gia tăng mỗi năm kể từ năm 2007 (UC Davis Health, 2021)
     
  • Tỉ lệ nỗ lực tự tử đã tăng gấp rưỡi trong số các cô gái tuổi dậy thì trong thời gian đại dịch coronavirus (Chiu, 2021)
     
  • Trung bình có khoảng 3,703 giới trẻ từ lớp 9-12 nỗ lực tự tử mỗi ngày tại Hoa Kỳ (CDC, 2020)

Biết được Dấu Hiệu

80% người muốn tự tử đưa ra các dấu hiệu về ý định tự tử của họ – nhận biết những dấu hiệu này thì thật là quan trọng để ngăn chặn.  

Những Yếu Tố Rủi Ro
  • Rối loạn tinh thần, cụ thể là rối loạn tâm trạng, tâm thần phân liệt, rối loạn lo âu và rối loạn nhân cách nhất định
  • Rối loạn do sử dụng rượu bia và các chất gây nghiện khác
  • Tuyệt vọng
  • Có khuynh hướng bốc đồng và/hay thù nghịch
  • Tiền sử về chấn thương hay lạm dụng
  • Bệnh nặng về thể chất
  • Những nỗ lực tự tử trước đây
  • Tiền sử tự tử của gia đình
  • Mất mát gần đây
  • Dễ tiếp nhận các phương tiện gây chết người
  • Các nhóm tự tử ở địa phương
  • Thiếu sự hỗ trợ của xã hội và cảm giác bị cô lập
  • Bị kỳ thị liên quan đến yêu cầu sự giúp đỡ
  • Thiếu sự chăm sóc y tế, đặc biệt là điều trị về lạm dụng chất gây nghiện và sức khoẻ tâm thần.
  • Niềm tin về tôn giáo và văn hoá, chẳng hạn như tin tưởng rằng tự tử là giải pháp cao quí cho tình trạng khó xử của cá nhân.
  • Tiếp xúc với những người đã chết do tự tử (trong đời sống thật hay thông qua truyền thông và Internet).
Những Dấu Hiệu Cảnh Báo
  • Nói về việc muốn chết hay tự sát
  • Tìm cách tự sát, như tìm trên mạng hay mua một khẩu súng
  • Nói về cảm giác tuyệt vọng hoặc không có lý do gì để sống nữa
  • Nói về cảm giác bị mắc kẹt hay nỗi đau không thể chịu đựng được
  • Nói về việc trở thành gánh nặng cho người khác
  • Gia tăng sử dụng rượu bia hay chất gây nghiện
  • Hành động lo lắng hay bị kích động; hành vi thiếu thận trọng
  • Ngủ quá ít hay quá nhiều
  • Tự rút lui hay tự cô lập
  • Thể hiện sự giận dữ hoặc nói về việc tìm cách trả thù
  • Tính khí thất thường
Làm thế nào để Giúp Đỡ

Nếu quí vị quan sát thấy có một hay nhiều dấu hiệu cảnh báo này, đặc biệt nếu hành vi mới xảy ra, đã gia tăng, hoặc dường như có liên quan đến một sự cố đau buồn, mất mát, hay thay đổi – vào cuộc hay lên tiếng.

Nói vài lời

Nhiều người trong chúng ta không cảm thấy thoải mái để nói về tự tử, và ngay cả lo sợ rằng nếu chúng ta nói ra vài lời có thể gây ra tình huống xấu hơn.  Nghiên cứu đã tho thấy rằng việc hỏi ai đó nếu họ có ý nghĩ tự sát thì thật ra có nhiều khả năng cứu sống một mạng người.  

  • Có trong tay danh sách các nguồn khủng hoảng.  Thực hành những điều quí vị sẽ nói.  Lập kế hoạch trao đổi vào thời gian quí vị không vội vàng và có thể dành thời gian cho người đó.  
     
  • Đề cập đến các dấu hiệu thúc dục quí vị hỏi về việc tự tử.  Điều này sẽ cho thấy rõ ràng rằng quí vị không hỏi “một cách bất ngờ” và khiến cho người đó khó có thể từ chối một vài điều đang làm phiền họ.
     
  • Hỏi trực tiếp về việc tự tử.  Nói về việc tự tử KHÔNG đặt ý tưởng đó vào đầu một ai và họ luôn cảm thấy nhẹ nhõm.  Hỏi trực tiếp và sử dụng từ “tự tử” để cho thấy rằng quí vị và người đang có nguy cơ cùng nói chuyện về điều tương tự và cho người đó biết rằng qui vị bằng lòng nói về tự tử. 
     
  • KHÔNG hỏi trong cách để cho thấy quí vị muốn có từ “không” cho câu trả lời.
     
  • KHÔNG bảo người đó làm điều đó.  Quí vị có thể muốn hét lên vì tức giận hay thất vọng, nhưng đây là điều nguy hiểm nhất mà qui vị có thể nói.  
  • KHÔNG bí mật hứa.  Người đó có thể nói rằng họ không muốn quí vị nói tới bất kỳ ai rằng họ đang muốn tự tử.  Thay vì đó hãy nói điều này: “tôi lo cho bạn quá nhiều khi phải giữ bí mật như thế này.  Bạn cần giúp đỡ và tôi đang ở đây để giúp bạn có được điều đó.” Quí vị có thể lo ngại rằng họ sẽ khó chịu với mình, nhưng khi tính mạng của ai đó gặp rủi ro, điều quan trọng là đảm bảo an toàn cho họ. 
Lắng Nghe

Đừng cố gắng giải quyết vấn đề của một ai hoặc bảo cho họ biết cảm giác của quí vị như thế nào.  Hãy để họ nói và thật sự lắng nghe những gì họ đang nói.  Hãy chắc chắn cho họ biết rằng quí vị đang ở đó với họ, rằng quí vị quan tâm đến họ, và rằng họ không đơn độc.    

  • Lắng nghe các lý do mà người đó có cho cả lúc sống và chết.  Xác nhận rằng họ đang cân nhắc cả hai lựa chọn và nhấn mạnh rằng sống là lựa chọn cho họ. 
     
  • Hãy để cho người đó biết quí vị quan tâm đến họ.  Để họ biết rằng quí vị xem xét tình huống của họ một cách nghiêm túc, và quí vị đang thật sự quan tâm đến họ, và quí vị sẽ nỗ lực để hỗ trợ họ trong một chặng đường dài. 

Tập Trung vào Vấn Đề An Toàn

Một khi quí vị đã mở ra kênh giao tiếp và lắng nghe họ, đây là thời gian để lập kế hoạch cho sự an toàn của họ. 

  • Hỏi người đó rằng họ đã tiếp cận tới bất kỳ phương tiện gây chết người nào chưa (vũ khí, thuốc thang, v.v.) và giúp loại bỏ chúng ra khỏi khu vực gần đó. (Có thể cần một người bạn khác, một người trong gia đình hay cơ quan thực thi pháp luật để giúp đỡ cho vấn đề này.)  Đừng tự đặt mình vào sự nguy hiểm; nếu quí vị lo lắng cho sự an toàn của chính mình, gọi 911.
     
  • Cùng tạo ra một kế hoạch an toàn.  Hỏi người đó điều gì sẽ giúp giữ họ an toàn cho đến khi họ tiếp xúc được một nhà chuyên môn. 
Nhận Sự Giúp Đỡ

Biết giới hạn về chuyên môn của quí vị và giúp người đó thấy giá trị của việc liên hệ với nhà chuyên môn để nhận sự trợ giúp.

  • Cung cấp cho người đó các nguồn mà quí vị đã chuẩn bị.  Gọi cho Đường Dây Nóng Ngăn Chặn Tự Tử Toàn Quốc bất kỳ lúc nào tại số 1-800-273-8255.
