Above and Beyond Recognition Award -
Nomination Form
The Sacramento City Unified School District Community Advisory
Committee (CAC) for Special Education, a committee of parents,
teachers and community members who advise the district on special
education issues, is seeking nominations for the 9th Annual
“ABOVE AND BEYOND” recognition awards.
This award recognizes parents, teachers, administrators, students,
classified employees, volunteers and community members who have made,
or are currently making, significant contributions to the district’s
Special Education programs.
If you know of an individual deserving such recognition, please
complete this form and return it to the Special Education Department
no later than Friday,
April 4, 2008.
An awards ceremony to honor the selected individuals is planned for
Tuesday evening, May 6, 2008 at 7:00 p.m. in the Community Meeting
Room of the Sacramento City Unified School District at the Serna
Center, 5735 47th Avenue, Sacramento (just west of Stockton
Boulevard).
