Certificates
of Insurance (or riders) are required by most non-District facilities when
space is being used for a school-sponsored event. To obtain a certificate of insurance, a Request for Certificate of Insurance Form must be requested from the
Risk Management Office, (916) 643-9421 and completed.
A written agreement and the contract or facility use permit
indicating insurance requirements of the facility should accompany the
completed form when it is submitted. Ten days before the event must be allowed to obtain the
certificate of insurance.
