An email sent on March 5, 2015 regarding reporting requirements spurred great conversation and interest from many of you. The Safe Schools Office, Legal and Risk Management would like to clarify a few issues as well as attach the revised forms recently completed by Risk Management.
In the event of an incident involving an injury (or potential injury, as we are not doctors and there are times when an injury may not be apparent at the time of the incident, such as a head injury), the Incident Form should be completed and forwarded to Risk Management and the Safe Schools Office (Safe Schools and Legal share copies of the incident).
If an incident involves a vandalism or theft of District-owned property, both forms attached should be completed because one form relates to liability and the second form relates to insurance.
