Preschool and Child Care Enrollment Requirements

Please Note:  To enroll in any of the programs, families will be required to bring the enrollment documents listed below with them when they enroll.

Preschool Programs

School Age - Free or Low cost Programs

School Age - Full Fee Program

Preschool Enrollment Requirements

Bring Child's:

·         Birth Certificate

·         Immunization Record (Bring original and one copy of front and back)

3 polio, 4 DTP, 3 Hep-B, 1 HIB 1 Varicella and 1 MMR
(Varicella & MMR need to be given after the child’s first birthday)

·         Medi-Cal Card and/or Medical Insurance Information

·         TB Clearance (Dated within 1 year of start of school)

·         Physical Examination with hemoglobin results (Dated within 1 year of start of school)

·         Dental Examination Results (Dated within 1 year of start of school)

·         WIC verification (if applicable)

Bring Adult's:

·         Verification of all sources of income (Most recent paycheck, disability, unemployment stubs, TANF verification, child support, etc.)

·         Address verification

·         TB Clearance (Dated within 1 year of start of school)
